$release is just the version of the Pkgfile itself. If you change the port in some way like changing build-options or adding patches without the $version chaning - then you should increment the $release. This is to differentiate the port.

The release number is used to make a difference in same port version, and force a rebuild. Doesn't matter how many patches you add. For a port with version X release Y, if you add some patches and you want to force a rebuild of the port, just use release Y+1 and in the next ports -u it will be shown and the port will be rebuilt.
